To assist you in securing retail financing for a truck / tractor / trailer purchase. I hereby authorize The Pete Store LLC and its financing partners to obtain my consumer credit report.

For the purpose of establishing and maintain credit, the undersigned submits the foregoing statement and information contained on this sheet, both written and printed, and including supplemental sheets, if any, as being a full, true and correct statement of my financial condition and all above matters, on the date stated. The undersigned agrees to notify you immediately in writing of any materially unfavorable change in my financial condition of the above matters, and in the absence of such notice or of a new and full written statement, all matters herein may be considered as continuing statement and substantially correct. The application hereby authorizes The Pete Store, LLC to make inquiry into, to request, and to receive any information concerning my character, general reputation, personal characteristics, mode of living, and all information from creditors which The Pete Store, LLC deems relevant for the granting and collection of the proposed borrowing. The authorization shall be effective from the fate upon which this application is signed and is extinguished automatically upon full payment of the present borrowing, if any is granted. Upon my written request, additional information as to the scope of this inquiry, if one is made, will be provided.

We request credit with The Pete Store group of dealerships (Peterbilt of Baltimore LLC, Peterbilt of Richmond Inc, Peterbilt of Knoxville Inc, Pete Store - Delaware LLC, The Peterbilt Store New England LLC) and/or Paclease of Baltimore LLC and agree to pay all invoices by the 10th of the following month. In the event that the account is not paid by the last day of the following month, I/We agree to pay a service charge of 1.5% per month (18% per year) on all such amounts, compounded monthly. I/We agree to pay for any collection costs, including court cost and attorney fees of not less than 33.33% of the total amount due.
